Macam-Macam Vitamin


A.KEKURANGAN VITAMIN A
1. Pengertian
Vitamin A merupakan suatu kondisi untuk menjaga kesehatan mata yang mana harus dilakukan sejak usia dini dengan mengkonsumsi buah-buahan, sayuran hijau, lauk hewani dan minyak ikan.

2. Gejala Klinis dan Tanda-tanda


Gejala pada mata disebut sebagai “Xeroftalmia” dan menurut WHO (1992) dibuat kriteria kelainan sebagai berikut :

§        Buta senja (Nigh Blindess, XN) yang diketahui bila anak sering jatuh atau salah        menangkap benda yang diberikan saat senja
§        Kekeringan pada konjungtiva (konjungtival xerosis, XIA), merupakan proses perubahan bulbus, yaitu kering, tebal, keriput dan terjadi penimbunan pigmen
§        Bercak bifot (Bifot Spot, XIB), berupa bercak warna putih berbuih dan terdiri dari penimbunan sel epitel
§        Kekeringan pada kornea (corneal ulcuration / keretomalacia) < 1/3 permukaan (x 3A), akibat keringnya epitel sehingga kejernihan kornea berkurang
§        Ulkus pada kornea (Cornea ulcuration / kerotomalacia) >1/3 permukaan (x 3B)
§        Xeroftalmia Fundus (XF)]

Defisiensi Vitamin A dianggap sebagai masalah kesehatan masyarakat bila suatu daerah menenuhi kriteria sebagai berikut :
§         X IB 0,5 % dari populasi yang memenuhi resiko
§         X2 + X3A + X3B 0,01 % dari populasi yang memenuhi resiko
§         X5 0,1 % dari populasi yang memenuhi resiko
§         XN 1 % dari populasi yang memenuhi resiko
§         Serum Vitamin A 10 mg / dl sebanyak 5 % dari populasi yang memenuhi resiko

3. Hal yang Harus Di Lakukan

Pada umumnya sehari-hari Vitamin A dapat dipenuhi pemberian diet yang mengandung telur, susu, mentega, hati, sayuran yang berupa daun atau berwarna kuning (wortel dan sebagainya) buah-buahan yang berwarna kuning (tomat, pepaya, dan sebagainya).

Pemberian Vitamin A dengan tujuan mengobati defisiensi vitamin A dan menambah persediaan Vitamin A dalam hepar, preparat yang dianjurkan adalah :

a.      Oral : oil – Based Solution Refind Pelmital atau asefat sebagai kapsul dengan / tanpa tambahan Vitamin E
b.      Infra maskuler : Water micsible refinol palmital

Pengobatan Xeroftalmia :

§         Setelah dibuat diagnosa
110 mg Refinol palmital atau 66 mg refinol asefat (200.000 SI) per oral, atau 55 mg refinol palmital (100.000 SI) intra vena
§         Hari berikutnya
110 mg refinol palmitat atau 66 refinol asefat (200.000 SI)
§         Sebelum di pulangkan / klinis memburuk / 2 – 4 minggu kemudian 110 mg refinol palmitat atau 66 mg refinol asetat (200.000 SI) per oral

Dalam pokok-pokok kegiatan program perbaikan gizi, distribusi kapsul Vitamin A diprioritaskan pada anak balita di daerah miskin dan atau kantung-kantung rawan KVA serta anak balita yang tinggal di daerah dengan angka mordibitas tinggi terutama, penderita campak dan diare serta ibu nifas

 B. KEKURANGAN VITAMIN C
1. Pengertian

Kekurangan vitamin C adalah suatu kondisi, dimana seseorang kekurangan vitamin C umumnya terdapat pada buah-buahan yaitu jeruk, belimbing dan sebagainya. Vitamin C diperlukan pada pembentukan zat kolagen oleh fibroblast hingga merupakan bagian dalam pembentukan zat intrasel. Keadaan kekurangan vitamin C akan mengganggu integritas dinding kapiler.
Vitamin C diperlukan juga pada proses pemetangan esitrosit dan pada pembentukan tulang dan dentin. Vitamin C mempunyai peranan penting pada respirasi jaringan .
Kekurangan vitamin C pertumbuhan anak terganggu dan timbul pendarahan kapiler dimana-mana, terutama didaerah periostium dekat tulang panjang.
Pada waktu anak dilahirkan persediaan vitamin C dalam tubuh cukup banyak, maka kejadian kekurangan vitamin C terjadi pada kebanyakan pada umur 6 sampai dengan 12 tahun, umur 1 tahun umumnya anak sudah dapat diet yang lebih berfariasi hingga angka kejadian menurun.
2. Gejala

1.      Cengang / mudah marah.
2.      Rasa nyeri pada tungkai bawah.
3.      Pseoduparalisis tungkai bawah, sedangkan tungkai atas jarang terserang.
4.      Pendarahan gusi.
5.      Sariawan.
6.      Bibir pecah-pecah.

Kelainan radiologis terdapat pada tulang-tulang yang sedang aktif tumbuh, seperti ujung sternum tulang rusuk, ujung distal femur, ujung proksimal humurus, kedua ujung tibia dan fibula, dan ujung distal radius dan ulna.
Gambaran radiologis menunjukkan adanya garis epifisis yang agak kabur dan tidak rata seperti biasa, osteoporosis ringan, pembengkakan pada ujung tulang panjang, terutama pada ujung bawah femur disebabkan oleh pendarahan subperios.
3. Penatalaksanaan


Pada umumnya kebutuhan sehari-hari vitamin C dapat dipenuhi dengan buah-buahan. Kekurangan vitamin C perlu dilakukan beberapa tindakan sebagai berikut:
1.      Pemberian makanan yang mengandung vitamin C.
2.      Cegah / atasi keseimbangan cairan dan pemberian cairan.
3.      Cegah / atasi rasa nyeri dan pemberian obat analgesik.
4.      Koreksi kadar vitamin C pada tubuh.
5.      Cegah / atasi BAB keras dengan pemberian serat.
6.      Terapiotik pemenuhan keseimbangan vitamin C.
C. KEKURANGAN VITAMIN D
1. Pengertian

Kekurangan vitamin D adalah suatu keadaan dimana seseorang kekurangan vitamin D, dimana vitamin D tersebut terdapat pada radiasi ersoterol secara artivisial dengan sinar ultra violet, contoh sinar matahari.
Apabila kekurangan itu terjadi pada masa pertumbuhan akan timbul osteomalasia dan mengakibatkan penyakit rathitis dan kadang-kadang terani.
Kekurangan vitamin D timbul klasifikasi tulang yang tidak normal, disebabkan oleh rendahnya saturasi kalsium dan fosfor dalam cairan tubuh.

Beberapa zat yang berhubungan dengan aktivitas vitamin D   adalah :
1.      Vitamin  D2 yang dihasilkan oleh radiasi ersoterol secara artivisial dengan sinar ultra violet.
2.      Vitamin D3 yang dihasilkan oleh radiasi pada kulit manusia dengan komponen ultra violet pada sinar matahari dan juga terdapat pada sumber makanan hewani.

2. Gejala

1.      Tubuh terasa pegal.
2.      Lunaknya tulang.
3.      Demineralisasi umum pada rangka.
4.      Deformitas torax, tulang punggung, pelvis dan tulang-tulang panjang.

Penatalaksanaan biokimia
a.       Kadar kalsium
b.       Kadar fosfor rendah
c.       Kadar fosfarase meninggi
d.       Kadar 25 (OH) vitamin D dibawah 4 mg/ml.
3. Penatalaksanaan



Umumnya kebutuhan sehari-hari vitamin D dapat terpenuhi dengan sinar matahari. Beberapa tindakan pada pasien kekurangan vitamin D sebagai berikut :

1.      Atasi / cegah kerapuan tulang dengan pemberian kalsium.


2.      Atasi / cegah radiasi pada kulit dengan pemberian komponen ultra violet. Yaitu sinar matahari.


3.      Atasi / cegah dehidrasi dengan pemberian cairan.


4.      Koreksi kadar kalsium pada tulang.


5.      Cegah infeksi dengan cara pemberian antibiotik.


6.      Fasilitas tumbuh kejar yaitu pada masa pemulihan dibutuhkan berbagai pendekatan dan kebutuhan vitamin D dengan terapiotik.
D. GANGGUAN AKIBAT KEKURANGAN IODIUM
1. Pengertian

Gangguan akibat kuranganya Iodium (GAKI) adalah sekumpulan gejala yang dapat ditimbulkan karena tubuh menderita kurang Iodium secara terus menerus dalam jangka waktu yang lama. Dampak negatifnya dapat berupa pembesaran kelenjar gondok, hipoteroid, kegagalan reproduksi kematian anak dan hambatan sosial ekonomi.
Iodium merupakan mineral yang penting dalam pembentukan horman teroid, pada keadaan normal kebutuhan pada anak-anak adalah : 200 mg/hari sumber utama Iodium adalah dari makanan dan absorbsi paling banyak terjadi di usus halus.
2. Gejala Klinis dan tanda-tanda

Kretin endemik terdapat di daerah gondok endemik, kelainan terjadi waktu bayi masih dalam kandungan atau tidak lama setelah dilahirkan dan terdiri atas kerusakan pada syaraf pusat dan hipotiroidisme.
Secara klinis kerusakan syaraf pusat bermanifestasi dengan :
§         Retardasi mental
§         Gangguan pendengaran sampai bisu tuli
§         Gangguan neuromotor seperti : gangguan bicara, cara jalan yang aneh, dsb
§         Hipoteroid dengan gejala :
a.          Miksedema pada hipotiroidisme
b.         Tinggi badan kurang cebol (stunted gruwth) dan osifikasi
3. Hal-Hal yang Harus di Lakukan
Kegiatan pokok penanggulangan GAKI meliputi :
§         Garam konsumsi yang beredar diseluruh Indonesia harus dalam bentuk garam ber Iodium dengan kadar yang telah ditetapkan yaitu : 30 – 80 PPM
§         Untuk meningkatkan konsumsi garam ber Iodium, lakukan mobilisasi sosial dengan pendekatan pemasaran sosial
§         Berikan suplementasi kapsul larutan minyak ber Iodium untuk daerah endemik sedang dan berat pada semua penduduk pria usia 0 – 20 tahun dan semua penduduk wanita usia 0 – 35 tahun, dengan dosis tertera pada tabel dibawah ini

Objectives

1. Explore what is involved in body piercing, tattoos, and cosmetic surgery
2. Discuss why people engage in these activities
3. Research the health risks associated with these activities
Materials

The class will need the following:
paper and pencils
newsprint and markers
computer with Internet access (optional but very helpful)
Procedures

1. Open the lesson with a discussion about current fashion trends that involve minor alterations to a part of the body. What trends are students attracted to? What trends do they think improve their appearance? List their ideas on a piece of newsprint. Their thoughts may include the following:
  • Dyeing their hair
  • Wearing tight pants
  • Wearing makeup
  • Piercing their ears
  • Wearing black
  • Wearing nail polish on their fingers and toes
  • Getting their hair styled
2. Tell students that during this lesson, they will examine three extreme fashion trends that involve altering the body and are difficult or impossible to undo. The trends are permanent tattoos, body piercing, and cosmetic surgery. To make each of these changes, people must either undergo a procedure or an operation.
3. Write each of these terms on the chalkboard or on a piece of newsprint. Discuss with the class what each one means; explanations follow:
  1. Tattoos.Created by an electric instrument made of needles, tattoos are permanent ink marks or designs. The needles penetrate the first two layers of the skin. A professional tattoo artist must have a permit from the local health department to operate a tattoo parlor.
  2. Body piercings.Piercings are holes put in the ears, nose, navel, eyebrows, lips, tongue, or other parts of the body for the purpose of putting jewelry in them. They must be done according to proper sanitary procedures. The area surrounding a piercing must be washed twice a day with antibacterial soap.
  3. Cosmetic surgery.Several forms of cosmetic surgery are discussed in this lesson: liposuction, breast enhancement, and rhinoplasty, often known as a nose job. Liposuction is a surgical procedure that removes fatty tissue in the legs, buttocks, abdomen, back, arms, face, neck, or other area of the body. Done properly, the procedure is safe and leaves only tiny scars. Breast enhancement involves inserting implants, usually bags filled with saltwater, into the breasts. Some risks are associated with this procedure, including rippling of the skin and the possibility of other health problems from an implant. Rhinoplasty is a surgical procedure that alters the nose’s size or shape. As with any operation, there are some risks involved, including a possibility that narrowing a nasal passage could result in breathing difficulties.
4. Divide students into pairs. Have each pair select one of the three procedures described in step 3 to research. After they’ve done the research, have each pair answer the questions that follow. Related Web sites are also listed. Questions for Each Group
  1. What is involved in getting a tattoo, having a body piercing done, or having cosmetic surgery performed? Describe how each is done.
  2. What are the benefits of engaging in this activity?
  3. What are the short-term health risks?
  4. What are the long-term health risks?
  5. Why do people engage in these activities? What is the attraction?
  6. Based on what you have learned, would you engage in this activity now or when you get older? Give reasons to support your ideas.

5. Give the students the rest of the class period to complete their research and answer the questions. During the next class period, have the pairs give a brief oral presentation to the class. Presentations should answer all the questions in step 4.
6. Discuss with students the findings of each pair of students. Ask students why they think young people are attracted to these activities, especially when so many health risks are involved. If one reason is so that young people can assert themselves as individuals, ask students to think of safer alternatives. How can young people make a statement about their individuality without hurting their bodies?
7. Conclude the lesson by discussing with the class what they learned about making good choices regarding their bodies. Help students understand that the key to making healthful choices is to have information that can be applied to their own lives. All the activities discussed in this lesson pose some health risks. Ask students whether learning about those risks changed their thinking about the activities. Knowing what they know now, would they engage in them?

1. What role does peer pressure play in encouraging young people to get tattoos and body piercings? Do young people feel they must do these things to be “cool”? Give reasons to support your ideas.
2. Imagine that you are with a group of friends who decide to get some tattoos. How would you handle the situation? Would you go along with your friends or refuse to go? Give reasons to support your ideas.
3. What current fashion trends do you think best express who you are? For example, would dyeing your hair red project the right image? Try to think of an activity that carries as few health risks as possible. Alternatively, think of other things you do that also express who you are. Consider what activities you enjoy or avoid, as well as what you talk about with friends and family.

Use the following three-point rubric to evaluate how well students can research different topics and answer assigned questions, apply what they learned to their own lives, and participate in class discussions on sensitive topics:

Fashion Trends of the Past
Have students research fashion trends of the past. Students can select any time period (for example, the flappers of the 1920s or the hippies of the 1960s). Have them answer the following questions:
  1. What kinds of clothes did people wear during the time period you selected?
  2. How did women wear their hair? How did men wear their hair?
  3. Did people engage in any behaviors that presented health risks? If so, were they aware of those risks? Did they change their behavior if they became aware of the risks?
  4. What do you think of the fashion trends from the time period you selected? Do you think they are attractive? Why or why not? Give reasons to support your ideas.
  5. Why do you think people may not find former fashion trends attractive? Why do you think fashion trends change?
  6. Did people do anything during the time you selected to permanently alter their bodies? If so, what?
